Abstract
Endokrini sustav dio je ljudskog organizma koji je odgovoran za procese neophodne za njegovo normalno funkcioniranje. Dio ovog sustava je štitna žlijezda koja putem hormona kontrolira metabolizam. Poremećaji rada štitne žlijezde mogu izazvati neurološke te psihološke simptome, premda nije utvrđena povezanost specifičnih sindroma i stanja štitnjače. Proveden je niz istraživanja koja su ispitivala povezanost poremećaja funkcije štitnjače i psihijatrijskih poremećaja, poglavito anksioznosti. Svrha ovog istraživanja je ispitati postoji li povezanost razina hormona štitnjače i anksioznosti kod osoba s tiroidnom disfunkcijom te kakve su posljedice na kvalitetu života pacijenata. U istraživanju je sudjelovalo 122 sudionika s bolestima štitnjače (108 žena i 14 muškaraca), raspona dobi od 18 do 73 godina. Pronađena je povezanost većine aspekata anksioznosti s brojem simptoma hipotireoze, hipertireoze i zajedničkih simptoma hipotireoze i hipertireoze. Veći broj simptoma općenito povezan je s povećanim aspektima anksioznosti, smanjenim mentalnim i tjelesnim aspektom kvalitete života i percipiranjem bolesti kao većim problemom. Nije pronađena povezanost razina hormona štitnjače s mjerama psihološkog funkcioniranja niti s brojem doživljenih simptoma. Osobe s bolestima štitnjače s više izraženih simptoma imaju lošije rezultate na mjerama psihološkog funkcioniranja. Nisu pronađene značajne razlike u psihološkom funkcioniranju između hipotiroidne, hipertiroidne i eutiroidne skupine sudionika. Faktori koji doprinose objašnjenju varijance mentalnog aspekta kvalitete života su: anksiozna osjetljivost, broj simptoma, uobičajeni simptomi anksioznosti, anksioznost kao stanje i osobina ličnosti. Dodatni faktor koji doprinosi objašnjenju varijance kriterija tjelesni aspekt kvalitete života je trajanje bolesti. Odnos većeg broja stresnih događaja te smanjenog mentalnog i tjelesnog aspekta kvalitete života posredovan je povišenom anksioznošću kao stanjem ili osobinom ličnosti. Ovi rezultati upućuju na ulogu broja doživljenih simptoma i anksioznosti u promjenama kvalitete života te ističu potrebu za daljnjim istraživanjem u svrhu poboljšanja kvalitete života osoba s bolestima štitnjače.
Abstract (english)
The endocrine system is a part of the human organism responsible for processes necessary for normal functioning. One part of this system is the thyroid gland which controls metabolism using hormones. Thyroid disorders can result in neurological and psychological symptoms, although the association between specific syndromes and thyroid conditions is not found. There has been numerous research examining association between thyroid and psychiatric disorders, especially anxiety. The aim of the current study was to investigate the relationship between thyroid hormones levels and anxiety in thyroid disorder patients and the consequences to patients quality of life. The study included 122 participants (108 females and 14 males), aged from 18 to 73. Most of the anxiety aspects were associated with number of hypothyroid, hyperthyroid and shared symptoms. Higher number of symptoms in general is associated with increased anxiety aspects, decreased quality of life aspects and perception of illness as a bigger problem. Thyroid hormone levels were not associated with psychological functioning measures nor with number of symptoms. Thyroid disorder patients with more symptoms show lower results on psychological functioning measures. There were no difference in psychological functioning between hypothyroid, hyperthyroid and euthyroid patients group. Factors explaining mental aspect of quality of life variance are: anxiety sensitivity, number of symptoms, usual anxiety symptoms, trait anxiety and state anxiety. In addition to those factors, physical aspect od quality of life variance is explained by duration of illness. Higher number of stressful events relation with decreased mental and physical aspect of quality of life is mediated by higher state or trait anxiety. The results of this study indicate the role which number of symptoms and anxiety have in affecting quality of life changes and highlight the need for further research with purpose to improve thyroid disorder patients' quality od life.
